Documentation needed before FIT assestment. Requirements:

  1. General practioner journals(since last medical examination or all when new applicant). On paper or on new USB-hub.
  2. Applicationform - See below.
  3. Journals from the hospital if have any(Not always stated on the ejournal / sundhedsjournalen(sundhed.dk)).
  4. Medical Certificate(Previous if have any). 
  5. Spectacle data(Brillerecept) from optician if have any.
  6. IMPORTANT!!! When you are booked You have accepted that DK/AME/80 are able to look up data on Sundhed.dk & FMK.
